GP Partner – Kidderminster Medical Centre

Kidderminster Medical Centre (KMC) is a GP Practice with a focus on “the whole patient” centred care. We are looking for a GP who would like to become (ideally) a 6-8 session Partner with us; someone who can demonstrate commitment and flexibility with the drive to help us optimise both patient care and our business.

KMC is part of Wyre Forest Health Partnership, which was formed in 2014. We have more than 50 GPs and 250 staff looking after 73 500 patients across 5 sites. We have the security of scale, but still with an identity and sense of team at our sites. As a large Partnership, we offer an excellent working environment with the opportunity to pursue your clinical interests and further professional development; we offer exceptional support and a good work/life balance. The WFHP Central Management Team also provides IT and HR services, robust financial scrutiny and operational support to the teams in the sites. As a fully integrated Primary Care Network (PCN), we have been able to maximise the patient care and other opportunities which PCN working offers. We can move resource around our sites to support each other when needed. In our PCN capacity we have given c115 000 Covid vaccinations whilst still seeing patients and offering good access to care. We are innovators, in both how we deliver care and organise our business.

Advanced Care Planning Practitioner

The Wyre Forest Health Partnership (WFHP) was formed in 2014 and currently consists of 5 well established surgeries based in and around the beautiful Wyre Forest area. We have 30 GP partners and in excess of 200 staff looking after over 73,000 patients. The motivation of joining forces and forming Wyre Forest Health Partnership was to keep traditional primary care values but in a new structure that improves and protects GPs and patient care well into the future. The post holder would have a varied workload and be part of a multi-disciplinary team and would be expected to work across all 5 WFHP sites

The role of Advanced Care Planning Practitioner is to support and enable permanent residents in all care homes (nursing, residential and learning disability) to discuss advance care planning.

The aim is for all new residents to be offered an initial review within 6 weeks of admission to a care home.

Any current residents identified by the care home, family or members of the primary care team can also be offered a review.

An annual holistic review of healthcare needs will be offered to all residents.
